Important Notes Regarding This Invoice
This Invoice has been prepared by the licensed Chemist team at Manila Chemist & Pharmaceutical Services, Inc., operating under the regulatory jurisdiction of the Bureau of Internal Revenue (BIR) and the Food and Drug Administration (FDA) of the Philippines. All pharmaceutical compounding and laboratory services rendered in compliance with Philippine national standards.
Please verify all line items against your service agreement. Any discrepancies must be reported within five (5) business days of the invoice date. The Chemist department reserves the right to re-issue a corrected Invoice if errors are identified.
All services were performed at our facility located in Philippines Manila and at the client's premises in Quezon City, Metro Manila. Travel and on-site supervision by the lead Chemist are included in the billed amounts above.
Terms and Conditions
1. Payment is due within thirty (30) calendar days from the date of this Invoice. Late payments shall incur a penalty of 1.5% per month as stipulated under Philippine commercial law.
2. All pharmaceutical products and compounded preparations are subject to the Philippine Food and Drug Administration (BFDA) regulations and the Pharmacy Act of the Philippines (Republic Act No. 10918).
3. The Chemist services described in this Invoice are non-transferable and specific to the named client. Subcontracting of services requires prior written consent.
4. This Invoice constitutes a valid tax document for the purposes of the Bureau of Internal Revenue (BIR) in the Philippines. An Official Receipt (OR) will be issued upon confirmation of payment.
5. Disputes arising from this Invoice shall be resolved through mediation in the courts of the Philippines, with jurisdiction in the city of Manila, Metro Manila.
6. All intellectual property related to proprietary compounding formulas developed by our Chemist team remains the exclusive property of Manila Chemist & Pharmaceutical Services, Inc.
