Important Notes & Terms of This Invoice
1. This invoice is issued by Al-Shifa Chemist & Pharmacy, a licensed chemist and pharmaceutical dispensing facility registered with the Saudi Ministry of Health and operating in Jeddah, Saudi Arabia. All products listed comply with Saudi Food and Drug Authority (SFDA) regulations.
2. The 15% Value Added Tax (VAT) applied on this invoice is in accordance with the Zakat, Tax and Customs Authority (ZATCA) of Saudi Arabia. The VAT registration number is printed in the header of this document for verification purposes.
3. All prescription medications dispensed by this chemist in Jeddah have been verified against valid prescriptions submitted by the purchasing medical facility. Cold-chain items (Item #3) were transported in temperature-controlled packaging and delivered within the required 2–8°C range.
4. Payment is due within fourteen (14) calendar days from the invoice date. Late payments will incur a penalty of 2% per month as per the commercial terms agreed upon between Al-Shifa Chemist and the purchasing entity.
5. Any discrepancies in quantity, product condition, or pricing must be reported to the chemist's billing department in Jeddah within five (5) business days of delivery. After this period, the invoice shall be considered accepted in full.

7. Al-Shifa Chemist & Pharmacy reserves the right to substitute any product with an equivalent therapeutic alternative if the original item is temporarily unavailable, provided prior written consent is obtained from the purchasing facility.
