  • This Invoice has been issued in accordance with the Swiss Federal Act on Therapeutic Products (ThG) and the Ordinance on the Dispensing of Medicines by a licensed Chemist in the Canton of Zurich.
  • All prescription items listed above were verified against valid prescriptions issued by a registered physician in Switzerland Zurich. The Chemist confirms that all medications were dispensed within their expiry dates and stored under appropriate conditions.
  • The insurance reimbursement portion (CHF 484.20) will be submitted directly to Krankenkasse Zürich AG by the Chemist within five (5) business days of this Invoice date. The patient is only responsible for the co-payment amount of CHF 16.30.
  • Insulin Glargine (Item 03) must be stored between 2°C and 8°C. The Chemist has provided a validated cold-chain transport container. Please do not freeze the medication.
