Invoice Payment Instructions & Important Notes
Payment Method: This Invoice is payable via wire transfer, ACH, or certified check. Please reference Invoice Number INV-HOU-2025-04872 on all remittances. Wire transfer details: Gulf Coast Chemical Laboratories, LLC, First National Bank of Houston, Routing No. 071000013, Account No. 4482-7719-003. For ACH payments, contact our accounts receivable department at (713) 555-0147, Option 2.
Payment Deadline: The full amount of this Invoice is due within thirty (30) calendar days of the Invoice date, no later than July 12, 2025. Late payments will be subject to a monthly interest charge of 1.5% as stipulated in the master service agreement dated January 15, 2024, between the Chemist and the client.
Scope Confirmation: All Chemist services listed in this Invoice were performed at the client's request and in accordance with the Statement of Work (SOW) attached to the master agreement. Any additional services performed beyond the original scope have been pre-approved in writing by the client's procurement department.
Regulatory Compliance: All laboratory analyses and field sampling conducted by the Chemist were performed in full compliance with the United States federal regulations, the State of Texas environmental statutes, and the municipal ordinances of the City of Houston. Certificates of analysis and chain-of-custody documentation are available upon request and are retained in the laboratory records for a minimum of seven (7) years as required by TCEQ.
Dispute Resolution: Any disputes regarding the charges on this Invoice must be submitted in writing to the undersigned Chemist within fifteen (15) business days of receipt. Unresolved disputes shall be governed by the arbitration clause in the master service agreement, with proceedings to be held in Harris County, Houston, Texas, United States.
