- All services rendered are provided by licensed dietitians registered with the Professional Regulation Commission (PRC) of the Philippines.
- Nutritional advice provided is for general wellness purposes and does not replace medical treatment. Clients with specific medical conditions should consult their physician.
- All nutritional assessments and meal plans are customized to accommodate Filipino dietary preferences, cultural practices, and local food availability in Metro Manila.
- Confidentiality of all client health information is maintained in accordance with the Philippine Data Privacy Act of 2012.
- Any changes to the scope of services must be agreed upon in writing by both parties.
- Late payments will incur a penalty of 1.5% per month on the outstanding balance.
- This invoice is valid for payment within ninety (90) days from the date of issue.
