Terms and Conditions – Plumbing Invoice (Chicago, United States)
1. This invoice represents a final accounting for all plumbing services and materials provided by the
undersigned plumber at the client's property in Chicago, Illinois, United States. All work was performed
in accordance with the Chicago Plumbing Code, Illinois State Plumbing Standards, and applicable OSHA
safety regulations.
2. The plumber, Windy City Plumbing & Drain Services, provides a one-year workmanship warranty on all
labor and a manufacturer's warranty on all installed parts and fixtures. This warranty is valid only for
the original client and the specific property address listed on this invoice.
3. All plumbing work performed in the City of Chicago requires a valid permit from the City of Chicago
Department of Buildings. The plumber has obtained and filed all necessary permits. The client is
responsible for any future code violations not caused by the work described herein.
4. This invoice is non-transferable. Payment is due from the named client only. The plumber reserves the
right to place a lien on the property under Illinois law (735 ILCS 5/9-101 et seq.) for any unpaid
balance exceeding sixty (60) days past due.
5. All prices are quoted in United States Dollars (USD). Tax rates reflect the combined Chicago, Cook
County, and Illinois state sales and use tax as of the invoice date. Rates are subject to change by
municipal or state authority.
