Important Notes & Terms
1. This Invoice is issued in accordance with the Israeli Tax Authority regulations and the Ministry of Education guidelines for contracted School Counselor services in the Israel Jerusalem district.
2. All counseling sessions described in this Invoice were conducted in full compliance with the Israeli Professional Counselors Association (IPCA) ethical standards and the school's student privacy policy.

4. Late payments on this Invoice will incur a monthly interest charge of 1.5% in accordance with Israeli commercial law (Section 10 of the Interest Law, 5713-1953).
5. This Invoice constitutes the final settlement for all School Counselor professional services rendered during the period of 01 May 2025 through 31 May 2025. No additional charges will be applied for this billing cycle.
6. The School Counselor, Dr. Miriam Levi, holds a valid license issued by the Israeli Ministry of Education and is registered with the Israel Jerusalem Regional Education Authority for the 2024–2025 academic year.
7. A copy of this Invoice has been filed with the school's administrative records and the provider's financial records in Israel Jerusalem. Both parties retain the right to request a duplicate copy within 90 days of the issue date.
