Payment Terms & Instructions
This Invoice is payable within thirty (30) calendar days of the issue date. Payment is due no later than July 15, 2025. Please remit payment via Electronic Funds Transfer (EFT) to the account below, or by cheque made payable to "Chen Community Wellness Practice." A late payment fee of 1.5% per month will be applied to outstanding balances after the due date, in accordance with the terms of our Service Agreement dated January 2025. All amounts are denominated in Canadian Dollars (CAD). This Invoice is issued in compliance with the tax regulations of the Province of British Columbia and the federal tax laws of Canada. The Social Worker services described herein are partially exempt from GST under the Canadian Excise Tax Act, Section 29, as they constitute qualifying social services delivered in Canada Vancouver.
