Payment Instructions & Notes
This Invoice is issued in accordance with the Social Worker service agreement (Contract No. UZ-TK-2025-0891) between Tashkent Social Welfare Agency and the Tashkent Municipal Social Protection Department. All services were rendered within the city of Tashkent, Uzbekistan, during the billing period specified above.
Payment of 47,288,000 UZS (Forty-Seven Million Two Hundred Eighty-Eight Thousand Uzbekistani Soms) is due within thirty (30) calendar days from the date of this Invoice. Please remit payment via bank transfer to the following account: Bank of Uzbekistan, Account No. 00012345678901234567, SWIFT: UZBKUZ2X, Branch: Tashkent Central.
All Social Worker services listed on this Invoice have been completed and verified by the supervising case manager. The Social Worker, Aziz Rahimov, holds a valid professional license issued by the Ministry of Labor, Employment and Social Protection of the Republic of Uzbekistan and operates in full compliance with national social work standards applicable in Tashkent and throughout Uzbekistan.
Terms and Conditions
1. This Invoice becomes payable upon receipt and acknowledgment by the client organization. Late payments shall incur a penalty of 1.5% per month as stipulated under Uzbekistan commercial law.
2. The Social Worker services described herein are non-refundable once the service period has commenced. Any cancellation must be communicated in writing at least seven (7) business days prior to the scheduled service date.
3. All client data and case records handled by the Social Worker during the provision of services in Tashkent, Uzbekistan, are subject to strict confidentiality as mandated by the Law of the Republic of Uzbekistan on Personal Data Protection.
4. Disputes arising from this Invoice or the underlying service agreement shall be resolved through the Tashkent City Arbitration Court in accordance with the procedural rules of Uzbekistan.
5. This Invoice is valid for one hundred twenty (120) days from the date of issue. After this period, the Social Worker services provider reserves the right to reissue the document with updated terms.
