Invoice Notes & Payment Instructions
1. This Invoice covers all professional Social Worker services rendered during the period of 01 May 2025 to 31 May 2025 within the administrative boundaries of Vietnam Ho Chi Minh City. All services were delivered in accordance with the Memorandum of Understanding signed on 01 January 2025 between the Social Worker and the Phu My Community Development Foundation.
2. Payment of this Invoice shall be made within fifteen (15) calendar days from the date of issue via bank transfer to the following account: Bank for Foreign Trade of Vietnam (Vietcombank), Ho Chi Minh City Branch – Account Name: Nguyen Thi Lan Anh – Account No: 0091001234567 – SWIFT Code: BFTVVNVX.
3. The Social Worker confirms that all services described in this Invoice were performed in full compliance with the Law on Social Work (Law No. 72/2006/QH11) and all applicable regulations of the Ho Chi Minh City People's Committee. All client data has been handled in strict accordance with Vietnam's Personal Data Protection Decree 13/2023/NĐ-CP.
4. Any disputes regarding this Invoice must be raised in writing within ten (10) business days of receipt. Failure to do so shall constitute acceptance of the Invoice as presented. All unresolved matters shall be subject to the jurisdiction of the People's Court of District 3, Ho Chi Minh City, Vietnam.
5. This Invoice is valid for a period of ninety (90) days from the date of issue. The Social Worker reserves the right to issue a revised Invoice should any clerical errors be identified within the first five (5) business days.
