Notes & Terms
This Invoice is issued in accordance with the Service Agreement dated March 1, 2025, between Haruki Tanaka (Special Education Teacher) and the Minato Ward Board of Education, Japan Tokyo. All services rendered during the billing period of April 1, 2025, through June 30, 2025, are itemized above.
The Special Education Teacher confirms that all professional services were delivered in full compliance with the standards set forth by the Tokyo Metropolitan Board of Education, the Japan Tokyo inclusive education framework, and the national Special Needs Education guidelines. All student records and confidential information have been handled in strict accordance with Japan's Personal Information Protection Act.
Should the client require a revised Invoice, a duplicate copy, or a detailed itemization of any line item, please contact the Special Education Teacher at the email or phone number provided above within 14 days of the Invoice date. After this period, the Invoice shall be considered final and binding.
This Invoice is valid for payment within 60 days of the issue date. The Japan Tokyo education authority reserves the right to request a formal audit of service hours and deliverables. The Special Education Teacher maintains complete time logs and session records available upon written request.
