Payment Terms & Conditions
This Invoice is issued in accordance with the professional services agreement (Contract No. MUSD-SE-2025-1142) between the Special Education Teacher, Dr. Margaret A. Whitfield, and the Manhattan Unified School District, operating within the jurisdiction of United States New York City.
- Payment is due within thirty (30) calendar days of the Invoice date, no later than July 15, 2025.
- Remittance shall be made via Electronic Funds Transfer (EFT) to the account designated on the provider's W-9 form on file with the District's Accounts Payable department.
- Late payments shall accrue interest at a rate of 1.5% per month, in compliance with New York State Education Law and applicable United States New York City municipal procurement regulations.
- All services rendered are subject to the standards set forth by the New York State Department of Education (NYSED) and the Individuals with Disabilities Education Act (IDEA).
- This Invoice constitutes the final billing for the service period of May 2025. Any additional services performed outside the contracted scope will be invoiced separately with prior written authorization.
Additional Notes
The Special Education Teacher services described in this Invoice were delivered in full compliance with all applicable federal, state, and local regulations governing special education in the United States New York City. All student records, IEP documentation, and behavioral intervention plans remain the property of the Manhattan Unified School District and are maintained in accordance with FERPA (Family Educational Rights and Privacy Act) and New York State Education Law Article 2.
Dr. Whitfield holds active certification as a Special Education Teacher (Grades 1–12) issued by the New York State Education Department, as well as Board Certification in Behavior Analysis (BCBA) through the Behavior Analyst Certification Board. All professional liability insurance is maintained at a minimum coverage of $2,000,000 per occurrence, as required by United States New York City Department of Education vendor policies.
