Terms and Conditions
1. This Invoice constitutes a formal request for payment for speech therapy services rendered by a qualified Speech Therapist in Melbourne, Australia. All services are provided in compliance with the Australian Privacy Act 1988 and the NDIS Quality and Safeguards Commission requirements.
2. Payment is due within fourteen (14) calendar days from the date of issue of this Invoice. Late payments may incur a late fee of 2% per month on the outstanding balance, in accordance with Victorian commercial tenancy and service billing regulations.
3. All speech therapy sessions are conducted by a registered Speech Therapist who holds current membership with Speech Pathology Australia and is registered on the Health Professionals Registration Board of Victoria (VHA) register.
4. Client records, therapy notes, and this Invoice document are stored securely in accordance with Australian data protection legislation. Records are retained for a minimum of seven (7) years as required by the Australian Health Practitioner Regulation Agency (AHPRA).
5. In the event of a dispute regarding this Invoice, parties agree to first attempt resolution through direct communication with the Speech Therapist practice manager in Melbourne, Australia, before pursuing formal dispute resolution through the Victorian Civil and Administrative Tribunal (VCAT).
