Important Notes Regarding This Invoice
1. This Invoice covers speech therapy services rendered between 01 May 2025 and 14 June 2025 at our Bangladesh Dhaka clinic facility.
2. All Speech Therapist sessions were conducted in person at our Banani, Bangladesh Dhaka location unless otherwise noted.
3. The 5% loyalty discount has been applied as per the annual family care plan agreement signed in January 2025.
4. VAT has been calculated at the standard 15% rate as mandated by the National Board of Revenue, Bangladesh.
5. Please retain this Invoice for your records. A digital copy has also been sent to the email address listed above.
6. Any queries regarding this Invoice or the speech therapy services provided should be directed to our billing department within 15 days of the issue date.
Terms and Conditions
- This Invoice is payable within fifteen (15) calendar days from the date of issue. Late payments shall attract a penalty of 1.5% per month as per Bangladesh commercial practice.
- All speech therapy services described in this Invoice have been delivered and are non-refundable once the session has been completed by the Speech Therapist.
- ClearVoice Speech Therapy Centre, Bangladesh Dhaka, reserves the right to reschedule sessions with a minimum of 24 hours' prior notice. Cancellations by the client within 12 hours of a scheduled session will be charged at 50% of the session fee.
- The Speech Therapist's clinical recommendations and progress reports are confidential and intended solely for the client and their authorized medical representatives.
- This Invoice does not constitute a guarantee of specific therapeutic outcomes. Speech therapy progress varies by individual, and the Speech Therapist will provide ongoing assessments to track development.
- Any disputes arising from this Invoice shall be resolved under the jurisdiction of the courts in Bangladesh Dhaka in accordance with the laws of the People's Republic of Bangladesh.
- By accepting this Invoice, the client acknowledges receipt of all services rendered and agrees to the payment terms outlined herein.
