Important Notes Regarding This Invoice
1. All Speech Therapy services described in this Invoice were delivered by a licensed Speech Therapist registered with the DR Congo Kinshasa Ministry of Health and the National Order of Speech-Language Pathologists.
2. The rates quoted reflect the standard professional fees for Speech Therapy services in the Kinshasa metropolitan area as of the 2025 fiscal year. No discounts have been applied.
3. This Invoice covers services rendered exclusively during the period of 01 May 2025 through 31 May 2025. Any additional sessions requested outside this period will be subject to a separate Invoice.
4. The client acknowledges that all Speech Therapy interventions were conducted in accordance with the ethical guidelines of the DR Congo Kinshasa Health Authority and international standards of practice.
5. A copy of this Invoice has been filed with the client's records under reference MKF-PT-2025-031. Please retain this document for your accounting and tax purposes in the Democratic Republic of Congo.
