Terms & Conditions – Speech Therapist Services in Iraq Baghdad
1. This Invoice represents the final billing for all Speech Therapist services rendered during the period of 02 June 2025 through 15 June 2025. All services were delivered by a licensed and certified Speech Therapist operating under the regulatory framework of the Iraqi Ministry of Health in Baghdad.
2. The Speech Therapist reserves the right to adjust session frequency, duration, or therapeutic approach based on the patient's clinical progress. Any additional sessions beyond the agreed treatment plan will be invoiced separately with a minimum 48-hour notice.
3. All home visits by the Speech Therapist within the Baghdad metropolitan area are included in the stated rate. Visits to locations outside Iraq Baghdad city limits (e.g., Hilla, Nasiriyah, Basra) will incur an additional travel surcharge of 20,000 IQD per visit.
4. Cancellations of scheduled Speech Therapist sessions must be made at least 24 hours in advance. Late cancellations or no-shows will be charged at 50% of the session fee as outlined in this Invoice.
5. All clinical records, progress reports, and diagnostic evaluations prepared by the Speech Therapist are the intellectual property of the provider and are shared with the client for personal and medical use only. Reproduction or distribution without written consent is prohibited.
6. This Invoice is governed by the laws of the Republic of Iraq. Any disputes arising from this Invoice or the associated Speech Therapist services shall be resolved through the competent courts in Baghdad, Iraq.
7. The Speech Therapist commits to maintaining the highest standards of clinical confidentiality in accordance with Iraqi medical ethics regulations. All patient data referenced in this Invoice and related documents are protected under applicable privacy legislation.
