Invoice Notes & Payment Instructions
Payment Method: Bank transfer to Al-Salam Speech & Language Therapy Center, National Bank of Kuwait, Account No. 0045-2218-7734, SWIFT: NBKWKWKA.
Reference: Please quote Invoice Number INV-KWT-2025-04782 in all correspondence and remittances.
Payment Due: This Invoice must be settled in full by 30 June 2025. A late payment surcharge of 2% per month will apply to any outstanding balance after the due date.
Insurance: The insurance deduction of 180.000 KWD has been pre-authorized by Bupa Kuwait under policy KWT-88234. The Speech Therapist's office will submit the remaining claim documentation directly to the insurer within 10 business days of this Invoice date.
Rescheduling: Should any future Speech Therapy sessions need to be rescheduled, please notify the Kuwait Kuwait City clinic at least 24 hours in advance to avoid a no-show fee of 15.000 KWD per session.
Terms & Conditions – Speech Therapist Services in Kuwait Kuwait City
1. This Invoice constitutes a formal request for payment for Speech Therapy services rendered by Dr. Layla Al-Mubarak, a licensed Speech Therapist registered with the Kuwait Ministry of Health, operating from the Al-Salam Speech & Language Therapy Center located in Kuwait Kuwait City.
2. All Speech Therapy services described in this Invoice were delivered in accordance with the treatment plan established during the initial assessment and approved by the client. The Speech Therapist reserves the right to modify the treatment plan based on the patient's progress, and any additional sessions beyond the scope of this Invoice will be billed separately.
3. The client acknowledges that the Speech Therapist in Kuwait Kuwait City adheres to the highest standards of clinical practice, confidentiality, and ethical conduct as mandated by the Kuwaiti healthcare regulatory framework. All patient records and session notes are maintained in compliance with Kuwait's data protection regulations.
4. This Invoice is valid for 60 days from the date of issue. After 60 days, the Speech Therapist's office reserves the right to suspend ongoing therapy services until the outstanding balance is cleared in full.
5. Disputes regarding the charges listed on this Invoice must be raised in writing within 14 days of the Invoice date. The Speech Therapist's office in Kuwait Kuwait City will review any disputed line items and respond within 7 business days.
6. The Speech Therapist is not liable for any indirect or consequential losses arising from the services described in this Invoice. The total liability of the Speech Therapist shall not exceed the total amount paid under this Invoice.
7. This Invoice and all associated Speech Therapy services are governed by the laws of the State of Kuwait. Any legal disputes shall be subject to the exclusive jurisdiction of the courts in Kuwait Kuwait City.
