Invoice Notes
This Invoice covers all Speech Therapist services rendered during the month of May 2025 at our Myanmar Yangon clinic located in Kyauktada Township. All sessions were conducted by Dr. Thiri Win Htet, a fully licensed and certified Speech Therapist registered with the Myanmar Ministry of Health and Sports.
The client is reminded that this Invoice must be settled within fifteen (15) calendar days from the Invoice date. Payment may be made via bank transfer to the account details below, or by cash at our Myanmar Yangon office during business hours (Monday to Saturday, 9:00 AM – 5:00 PM).

Terms & Conditions
1. This Invoice is valid for thirty (30) days from the date of issue. Late payments may incur a penalty of 2% per month as per Myanmar commercial regulations.
2. All Speech Therapist services described in this Invoice were delivered in accordance with the treatment plan agreed upon at the initial consultation. No additional services were performed beyond the scope outlined herein.
3. The Speech Therapist reserves the right to modify the treatment plan based on the client's progress. Any changes to the service schedule will be communicated in writing and a supplementary Invoice will be issued if additional sessions are required.
4. All records, assessments, and progress reports generated by the Speech Therapist remain the property of the Myanmar Yangon clinic. Copies may be provided to the client or their designated school upon written request.
5. This Invoice is subject to the laws of the Republic of the Union of Myanmar. Any disputes arising from this Invoice shall be resolved through the competent courts in Yangon, Myanmar.
6. The Speech Therapist services are covered under the clinic's professional liability insurance. In the event of a claim, the client should notify the clinic in writing within fourteen (14) days of the incident.
