Important Notes Regarding This Invoice
1. This Invoice covers all Speech Therapist services rendered during the month of June 2025 at our clinic located in Riyadh, Saudi Arabia. All sessions were conducted in compliance with the Saudi Ministry of Health guidelines for speech and language pathology.
2. The Speech Therapist assigned to this case, Dr. Sarah Al-Otaibi, holds a Master's degree in Speech-Language Pathology and is certified by the American Speech-Language-Hearing Association (ASHA) as well as registered with the Saudi Commission for Health Specialties (SCFHS).
3. All Speech Therapy services described in this Invoice were delivered in person at our Riyadh facility or via authorized home visits within the Riyadh metropolitan area. No telehealth sessions are included in this billing period.
4. The 15% Value Added Tax (VAT) applied on this Invoice is in accordance with the Zakat, Tax and Customs Authority (ZATCA) regulations in Saudi Arabia. A VAT-compliant receipt will be issued upon full settlement of this Invoice.
5. Should the client require additional Speech Therapy sessions beyond the scope of this Invoice, a supplementary Invoice will be generated and communicated via email within 48 hours of the additional service delivery.
6. This Invoice is valid for 30 days from the issue date. After this period, the Speech Therapist team reserves the right to reschedule remaining sessions or adjust the treatment plan accordingly.
Terms & Conditions
By accepting this Invoice, the client acknowledges and agrees to the following terms governing the Speech Therapist services provided by Al-Noor Speech & Language Clinic in Riyadh, Saudi Arabia:
(a) All Speech Therapy services are subject to the professional judgment of the treating Speech Therapist. Treatment plans may be modified based on clinical progress without prior written consent, though the client will be notified of any significant changes.
(b) Cancellations of scheduled Speech Therapy sessions must be made at least 24 hours in advance. Sessions cancelled with less than 24 hours' notice will be billed at 50% of the session rate as stated in this Invoice.
(c) The Speech Therapist and clinic staff are bound by strict confidentiality under Saudi data protection laws. All client records, assessment reports, and therapy notes remain the property of the clinic and will not be disclosed to third parties without written consent, except where required by Saudi law.
(d) Disputes arising from this Invoice or the associated Speech Therapy services shall be resolved through the competent courts in Riyadh, Saudi Arabia, in accordance with the Saudi Civil Transactions Law.
(e) This Invoice constitutes the complete and final billing document for the services rendered during the stated period. No additional charges will be applied beyond the amounts specified herein unless a separate written agreement is executed by both parties.
