Invoice Notes
This Invoice covers all speech therapy services rendered by Dr. Grace Nakato Mubiru, a registered Speech Therapist operating in Uganda Kampala, for the period of 2 June 2025 to 13 June 2025. All services were delivered in accordance with the treatment plan agreed upon during the initial consultation.
The Speech Therapist confirms that all sessions were conducted in full compliance with the Uganda Nursing and Midwifery Council (UNMC) guidelines and the standards set by the Uganda Association of Speech-Language Pathologists.
Please note that this Invoice is valid for 30 days from the date of issue. Any queries regarding this Invoice should be directed to the Speech Therapist's office at the Kampala Speech & Language Centre, Kampala Road, Uganda Kampala.
Payment Terms and Instructions
- Payment Due: Within 15 calendar days from the Invoice date (by 30 June 2025).
- Payment Method: Bank transfer to Kampala Speech & Language Centre, Bank of Uganda, Account No: 0012-3456-7890, Branch: Kampala Road.
- Reference: Please quote Invoice No. ST-UG-2025-0472 in all payment communications.
- Late Payment: A late payment surcharge of 2% per month will be applied to any outstanding balance after the due date, in accordance with Uganda commercial law.
- Currency: All amounts are stated in Ugandan Shillings (UGX). International payments will be converted at the Bank of Uganda's daily exchange rate.
- Disputes: Any discrepancies on this Invoice must be reported in writing to the Speech Therapist within 7 days of receipt. Undisputed amounts remain payable on the original due date.
