Payment Terms & Conditions
- This Invoice is payable within 15 days from the date of issue, i.e., by 30 June 2025, via NEFT/RTGS to the bank account mentioned below.

- A late payment penalty of 1.5% per month (or part thereof) shall be applicable on the outstanding Invoice amount after the due date.
- All services rendered by the Statistician are subject to the Master Service Agreement dated 01 March 2025 between DataStat Analytics Pvt. Ltd. and Meridian Healthcare Research Institute, both registered in India Bangalore.
- This Invoice is valid for 90 days from the date of issue. Any dispute regarding the Invoice must be raised in writing within 7 working days.
- GST has been charged as per the applicable rates under the Goods and Services Tax Act, 2017, for services rendered in India Bangalore, Karnataka.
Notes & Additional Information
1. The Statistician, Dr. Rajesh Venkatesh, holds a Ph.D. in Biostatistics from the Indian Statistical Institute, Kolkata, and has over 14 years of experience in clinical trial statistics. All analyses were performed using R (v4.3.2), SAS 9.4, and Python (v3.11) in compliance with 21 CFR Part 11 and CDISC SDTM/ADaM standards.
2. All raw data, code scripts, and intermediate outputs related to this Invoice are stored on the secure server at DataStat Analytics, India Bangalore, and will be retained for a minimum of 10 years as per regulatory requirements.
3. The client is requested to verify the line items on this Invoice against the approved Statement of Work (SOW) reference number SOW/MHR/2025/019. Any discrepancies should be communicated to the billing department immediately.
4. This Invoice does not include travel, accommodation, or per-diem expenses incurred by the Statistician during on-site visits to India Bangalore. Such expenses, if any, will be billed separately with supporting documentation.
5. DataStat Analytics Pvt. Ltd. is a registered company under the Companies Act, 2013, with its principal place of business in India Bangalore. This Invoice is issued in accordance with Section 31 of the CGST Act, 2017.
