Important Notes and Payment Information
This Invoice is issued by Pacific Paws Veterinary Clinic, a licensed Veterinarian practice operating in Canada Vancouver, British Columbia. All services were rendered in accordance with the standards set forth by the British Columbia Veterinary Medical Association and the Canadian Veterinary Medical Association.
Please remit payment of the outstanding balance of $773.90 CAD by the due date of July 14, 2025. Payment may be made via Interac e-Transfer to [email protected], by credit card (Visa, MasterCard, Amex), or by cheque payable to "Pacific Paws Veterinary Clinic" mailed to our Vancouver, Canada address listed above.
A late payment fee of 1.5% per month will be applied to any balance remaining after the due date, in compliance with the Canada Vancouver commercial billing regulations. Please reference Invoice Number PPV-2025-00487 with all correspondence and payments.
The Veterinarian recommends a re-examination in 12 months (June 2026) for the next annual wellness visit. A follow-up dental check is advised in 6 months to monitor healing of the extracted premolar (tooth #208).
