Important Notes Regarding This Invoice

This Invoice has been issued by Clínica Veterinaria Andina, a registered and licensed veterinary practice operating in Chile Santiago under the supervision of Dr. Carlos Andrés Muñoz Silva, a certified Veterinarian with over seven years of clinical experience in small animal medicine and orthopedic surgery.

Terms and Conditions

1. This Invoice is valid for 30 days from the date of issue. Late payments may incur a monthly interest rate of 1.5% in accordance with Chilean commercial law.

2. All veterinary services described herein were performed by a licensed Veterinarian at our facility in Chile Santiago. The client acknowledges receipt of informed consent prior to the surgical procedure.

3. Medications dispensed must be administered strictly according to the Veterinarian's written instructions. Any adverse reactions should be reported to the clinic immediately.

4. This Invoice does not constitute a guarantee of surgical outcome. The Veterinarian team has exercised the standard of care expected in the veterinary profession in Chile Santiago.

5. For disputes or billing inquiries, please contact the clinic management within 15 business days of the Invoice date.

6. This document is issued in accordance with the Chilean tax regulations (SII) and the professional standards of the Colegio de Médicos Veterinarios de Chile.
