Payment Terms and Important Notes
1. This Invoice is issued by Beijing Capital Veterinary Medical Center, a registered and licensed Veterinarian practice operating under the regulations of the Beijing Municipal Commission of Agriculture and Rural Affairs, China Beijing. All services rendered comply with the Veterinary Drug Administration Regulations of the People's Republic of China.
2. Payment is due within fourteen (14) calendar days from the Invoice date. Accepted payment methods include: UnionPay bank transfer, Alipay, WeChat Pay, and cash (RMB only). For corporate or institutional clients, a formal remittance to our bank account at ICBC Beijing Chaoyang Branch (Account: 0200 0045 0908 8812 345) is preferred.
3. The VAT rate of 6% applied on this Invoice is in accordance with the Value-Added Tax Law of the People's Republic of China for medical and veterinary services. A VAT special invoice (fapiao) will be issued upon request for tax deduction purposes.
4. The pet owner acknowledges that the surgical procedure described in Ref #005 was performed after informed consent was obtained and documented. The lead Veterinarian, Dr. Wei Zhang, and the surgical team bear professional responsibility for the procedure in accordance with the Veterinary Practitioners' Code of Ethics.
5. Follow-up appointments are mandatory at 14 days and 6 weeks post-surgery. Failure to attend scheduled follow-ups may affect the warranty on the orthopedic implant and the Veterinarian's post-operative care obligations.
6. This Invoice is valid for 90 days from the date of issue. After this period, a late payment surcharge of 1.5% per month will be applied. Disputes regarding this Invoice must be submitted in writing to the billing office within 30 days.
7. All medical records, diagnostic images, and surgical reports associated with this Invoice are retained at our China Beijing facility for a minimum of ten (10) years as required by national veterinary record-keeping regulations.
