Terms and Conditions of this Invoice:
1. This Invoice is issued by GreenPaws Veterinary Clinic Pte Ltd, a registered Veterinarian practice in Singapore Singapore, and is governed by the laws of Singapore Singapore.
2. Payment is due within fifteen (15) calendar days from the Invoice date. Late payments shall attract interest at 2% per month as per the Late Payment of Outstanding Debts Act (Singapore Singapore).
3. All Veterinarian services rendered are subject to the clinical judgement of the attending Veterinarian. No refunds will be issued for services already performed.
4. Insurance claims must be submitted directly by the client to their respective insurer. This Invoice serves as the official documentation for all Veterinarian procedures performed.
5. This Invoice is valid for a period of ninety (90) days from the date of issue. After this period, the Invoice may be reissued with applicable administrative charges.
6. All medical records and treatment plans associated with this Invoice are maintained in accordance with the Animal and Veterinary Act (Singapore Singapore) and the Personal Data Protection Act.
7. For disputes regarding this Invoice, please contact our billing department within fourteen (14) days. A copy of this Invoice must accompany any written dispute.
