Terms & Conditions of This Invoice
1. This Invoice constitutes a formal request for payment for veterinary services rendered by our Veterinarian team at Thames Valley Veterinary Clinic, London, United Kingdom. By accepting the services, the client agrees to the terms set out herein.
2. All prices are quoted in British Pounds Sterling (GBP) and are inclusive of the applicable United Kingdom Value Added Tax (VAT) at the standard rate of 20%, as required by HM Revenue & Customs (HMRC) for veterinary services provided in London.
3. Late payment of this Invoice will incur a statutory interest charge of 4% above the Bank of England base rate, calculated from the due date until the date of full settlement, in accordance with the Late Payment of Commercial Debts (Interest) Act 1998.
4. The Veterinarian services described in this Invoice were performed to the highest professional standard in compliance with RCVS guidelines. Any disputes regarding the clinical appropriateness of the services should be raised in writing within thirty (30) days of the Invoice date and will be reviewed by our Clinical Director, Dr. Sarah M. Okafor, BVetMed, MRCVS.
5. This Invoice is valid for a period of twelve (12) months from the date of issue. After this period, the Invoice may be reissued with any applicable administrative surcharge as determined by the practice.
6. All personal data processed in connection with this Invoice is handled in accordance with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018. Our practice is registered with the Information Commissioner's Office (ICO) under registration number ZB4421987.
