Payment Terms & Instructions
This Invoice is payable within thirty (30) calendar days from the Invoice date of June 12, 2025. A 2% early payment discount applies if the full balance is remitted on or before June 25, 2025. A late payment fee of 1.5% per month will be assessed on any outstanding balance after the due date of July 12, 2025.
Payment Methods Accepted: Wire transfer (ACH), check payable to "Midwest Visual Productions LLC," or corporate credit card (Visa, Mastercard, Amex). For wire transfers, please reference Invoice Number MVP-2025-00472 in the memo field.

Additional Notes & Terms
1. All videographer services described in this Invoice were performed in accordance with the Master Service Agreement (MSA) dated March 3, 2025, between Midwest Visual Productions LLC and Northshore Corporate Events Group.
2. The videographer team holds full liability insurance ($2,000,000 general liability) and workers' compensation coverage, compliant with all Illinois state and Chicago municipal regulations for commercial filming operations.
3. All footage and deliverables become the exclusive property of the client upon full payment of this Invoice. Midwest Visual Productions LLC retains the right to use up to 30 seconds of footage in its own portfolio and marketing materials unless otherwise specified in writing.
4. This Invoice is subject to the laws of the State of Illinois. Any disputes arising from this Invoice shall be resolved through binding arbitration in Cook County, Chicago, United States, in accordance with the rules of the American Arbitration Association.
5. Midwest Visual Productions LLC is a registered limited liability company in the State of Illinois, operating primarily in the United States Chicago metropolitan area and surrounding regions. All services are performed by licensed and insured videographer professionals.
