Invoice Notes
This Invoice covers all Welder services rendered by Andina Welding Solutions S.A.C. at the client's industrial facility located in Peru Lima during the period of June 2 through June 14, 2025. The assigned Welder, Sr. Carlos Huamán, holds valid AWS and ASME certifications and operated in full compliance with Peruvian occupational health and safety regulations (Ley 29783).
All welding work performed under this Invoice was executed in accordance with the approved Welding Procedure Specifications (WPS) and met the acceptance criteria defined in the project contract. The Welder maintained daily production logs and quality records, which are available upon request.
Payment is due within 30 calendar days from the date of this Invoice. Please reference Invoice number AW-2025-00487 in all correspondence and remittances. Bank transfer details: BCP, Cuenta Corriente 191-19045678-0-19, CCI: 002-191-000000000000000000-19.

Terms and Conditions
1. This Invoice is valid for 30 days from the date of issue. Late payments will accrue interest at the rate established by the Superintendencia de Banca, Seguros y AFP (SBS) of Peru.
2. All Welder services described in this Invoice were performed at the client's premises in Peru Lima. The client is responsible for providing safe access to the work area and ensuring all site-specific safety protocols are communicated to the Welder team prior to commencement of work.
3. Warranty: Andina Welding Solutions S.A.C. provides a 12-month warranty on all weld joints covered by this Invoice, provided the welded structures are not subjected to loads or conditions exceeding the design specifications. Any warranty claims must be submitted in writing within the warranty period.
4. This Invoice does not include additional Welder labor hours, expedited work, or services beyond the scope defined in the original contract. Any additional work will be billed separately via a supplementary Invoice.
5. All prices are quoted in Peruvian Soles (PEN) and are inclusive of the Welder's labor, standard equipment, and consumables as itemized above. Travel, lodging, or per diem for the Welder beyond the standard Peru Lima metropolitan area is not included unless previously agreed in writing.
6. Disputes arising from this Invoice shall be resolved under the jurisdiction of the courts of Lima, Peru, in accordance with the Peruvian Civil Code and applicable commercial legislation.
