Invoice Notes & Welder Certification Details
This Invoice is issued by Midwest Precision Welding Co., a licensed welding contractor operating in Chicago, United States. All welding services described herein were performed by certified welders holding current AWS (American Welding Society) certifications. Lead Welder Marcus J. Delgado holds the AWS Certified Welding Inspector (CWI) credential, Certificate No. 2019-CWI-88412, valid through December 2026.
All welds were executed in strict compliance with the applicable AWS D1.1 structural welding code and ASME Section IX qualification requirements. The welding procedures were reviewed and approved by the client's structural engineer prior to commencement of work at the Chicago, United States project site.
Payment for this Invoice is due within thirty (30) calendar days of the Invoice date. A 2% early payment discount is available if the full amount is remitted within ten (10) days of the Invoice date. Late payments are subject to a 1.5% monthly finance charge as permitted under Illinois commercial code 815 ILCS 205/1.

Terms and Conditions
1. This Invoice constitutes a binding request for payment for welding services rendered in the Chicago, Illinois, United States jurisdiction. All work was performed in accordance with the signed Statement of Work dated May 28, 2025.
2. Midwest Precision Welding Co. maintains Comprehensive General Liability insurance of $2,000,000 per occurrence and Workers' Compensation coverage for all welders and technicians as required by the State of Illinois.
3. All welds carry a 12-month workmanship warranty from the date of final inspection sign-off. The warranty covers defects in weld penetration, porosity, and structural integrity under normal service conditions.
4. Disputes arising from this Invoice shall be resolved through mediation in Cook County, Illinois, United States, in accordance with the American Arbitration Association commercial rules.
5. This Invoice is valid for 90 days from the date of issue. After 90 days, the Invoice may be subject to re-evaluation of applicable tax rates and currency adjustments.
