This document aims to outline recent findings regarding these shifts within the dense urban environment of Johannesburg.

Johannesburg presents a unique microcosm for this study due to its stark socioeconomic disparities. While the city boasts world-class private healthcare infrastructure, the majority of its population relies on an overstretched public health system. As a Medical Researcher operating in this context, one must navigate not only clinical data but also social determinants of health that are uniquely concentrated in Johannesburg's townships and inner-city informal settlements. The intersectionality of poverty, HIV/AIDS prevalence, and emerging NCD rates requires a nuanced research approach that distinguishes Johannesburg from other urban centers globally. Understanding these specific dynamics is critical for developing targeted interventions that can withstand the pressures of a high-density, resource-constrained setting.

The primary objective of this study was to quantify the rising prevalence of NCDs among adults aged 35 and older in Johannesburg, South Africa. Specifically, we sought to determine the correlation between urban lifestyle factors—such as diet, physical activity levels, and stress—and clinical markers for hypertension and type 2 diabetes. Furthermore, this research aimed to evaluate the accessibility of screening services within public health clinics in key Johannesburg districts including Soweto, Alexandra Township (Alex), and central Johannesburg.

The Urban Context: Johannesburg as a Research Site

Johannesburg is characterized by rapid urbanization, which has led to significant changes in lifestyle behaviors. The shift from traditional physical labor to service-oriented jobs, combined with the increased availability of processed foods in convenience stores within low-income areas, has contributed to higher rates of obesity and metabolic syndrome. As a Medical Researcher focusing on this region, it is essential to recognize that Johannesburg’s infrastructure plays a pivotal role in shaping these health behaviors. The "food desert" phenomenon is less prevalent than the "food swamp" effect, where unhealthy options are more accessible and affordable than nutritious alternatives.

HIV-NCD Comorbidity
