Introduction to Occupational Therapy in Uganda

Occupational Therapy is defined as a client-centered health profession concerned with promoting health and well-being through occupation. In Kampala, where urbanization is outpacing infrastructure development, OTs are uniquely positioned to address barriers created by both physical environments and societal structures. This document highlights the necessity of integrating OT into standard care pathways in Ugandan hospitals, community health centers, and NGOs operating across the capital city.

Contextual Challenges in Kampala

Kampala presents unique challenges for rehabilitation. The city faces a dual burden of disease, grappling with high rates of road traffic accidents due to urban transport issues, as well as a rising prevalence of non-communicable diseases such as diabetes and hypertension. Furthermore, historical conflicts in the northern regions have resulted in a significant population requiring psychosocial rehabilitation.

Key Barriers Identified:

• Limited Accessibility : A significant portion of Kampala’s infrastructure lacks ramps, tactile paving, or accessible public transport, making community reintegration difficult for individuals with mobility impairments.
• Workforce Shortages : There is a critical shortage of qualified Occupational Therapists in Uganda. Most services are concentrated in Mulago National Referral Hospital and a few private clinics in central Kampala, leaving vast peri-urban areas underserved.
• Cultural Stigma : In many communities surrounding Kampala, disability is still misunderstood or stigmatized. This affects the uptake of rehabilitation services and the social inclusion of patients.
• Economic Constraints : High out-of-pocket costs for assistive devices (wheelchairs, walkers) limit access for low-income families residing in informal settlements like Kisenyi or Katwe.

Strategic Interventions & Methodology

To address these challenges, this presentation advocates for a multi-faceted approach to Occupational Therapy practice in Kampala. The methodology focuses on three pillars: Clinical Excellence, Community Empowerment, and Advocacy.

1. Clinical Excellence & Education

We propose expanding OT training programs within Ugandan universities to produce more locally trained practitioners who understand the local context. Current curricula must be adapted to include low-resource settings strategies, where therapists learn to create adaptive devices using locally available materials.

2. Community-Based Rehabilitation (CBR)

Moving beyond hospital walls, OTs in Kampala should lead CBR initiatives. This involves training community health workers and family caregivers on how to support activities of daily living (ADLs). For example, teaching modified techniques for cooking or dressing that accommodate physical limitations without requiring expensive equipment.

3. Policy Advocacy

Occupational Therapists must engage with the Ministry of Health in Uganda to integrate OT services into National Health Insurance schemes (NHIF). This financial support mechanism would drastically improve access for the urban poor in Kampala.

Case Study: Road Traffic Accident Rehabilitation

Road traffic accidents are a leading cause of disability in Kampala. A retrospective analysis of patients treated at a central Kampala clinic over two years reveals the impact of OT.

• Patient Profile : Middle-aged male, motorcyclist involved in collision resulting in lower limb amputation.
• OT Intervention : Counseling on grief and adjustment; training in prosthetic use; home environment assessment to remove trip hazards; liaison with local artisans for customized footplates.
• Outcome : Patient returned to work as a bicycle taxi rider (boda-boda) with modified controls, achieving economic independence within six months. Family reported improved mental health and social participation.
