This Purchase Order is issued by the Centre Hospitalier Universitaire de Algiers to formally engage the services of a qualified Dietitian for the provision of clinical nutritional assessment, dietary planning, and ongoing patient follow-up within the metropolitan area of Algeria Algiers. The Dietitian shall operate in accordance with the professional standards established by the Algerian Ministry of Health and the National Order of Dietitians of Algeria.

All prices are quoted in Algerian Dinars (DZD). The Dietitian shall provide all services at the CHU Algiers facility located in the city of Algiers, Algeria, or at such other location within Algeria Algiers as mutually agreed in writing.

The Dietitian engaged under this Purchase Order shall fulfill the following obligations:

• Provide all nutritional services in a professional, ethical, and culturally sensitive manner appropriate to the diverse patient population of Algeria Algiers, including consideration of local dietary traditions, religious fasting periods (Ramadan), and regional food availability.
• Maintain full confidentiality of all patient medical and nutritional records in compliance with Algerian data protection regulations and the professional code of ethics of the National Order of Dietitians.
• Coordinate directly with the attending physicians, nurses, and hospital kitchen staff at CHU Algiers to ensure that all therapeutic diets are accurately prepared and delivered.
• Submit all monthly reports and progress documentation to the Head of Clinical Nutrition within five (5) business days of the end of each calendar month.
• Hold valid professional registration with the Algerian health authorities and maintain professional liability insurance for the duration of this Purchase Order.
• Be available for on-site consultation at the CHU Algiers facility during standard working hours (Monday to Friday, 08:00–17:00) and on a rotating weekend duty schedule as required by the hospital.

Payment for the services rendered under this Purchase Order shall be made in Algerian Dinars (DZD) via bank transfer to the account of the Dietitian's registered practice. The payment schedule is as follows:

• First installment (40%): DZD 812,056 – due within fifteen (15) calendar days of the date of this Purchase Order, upon receipt of a valid invoice.
• Second installment (35%): DZD 710,549 – due upon completion of the third month of service delivery, subject to satisfactory performance review.
• Final installment (25%): DZD 507,535 – due within ten (10) calendar days of the final monthly report submission and acceptance by the CHU Algiers medical board.

Any payment not received within the stipulated period shall accrue a late penalty of 1% per month, in accordance with Algerian commercial law. The Dietitian shall issue a detailed invoice for each installment, referencing this Purchase Order number (PO-DA-2025-04782).

This Purchase Order shall remain in effect for a period of six (6) months from the date of issue, i.e., from 15 June 2025 to 15 December 2025, unless renewed in writing by both parties. Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of termination, the Buyer shall be liable for all services rendered up to the effective date of termination.

This Purchase Order shall be governed by and construed in accordance with the laws of the People's Democratic Republic of Algeria. Any disputes arising from this Purchase Order shall be resolved through the competent courts of Algiers, Algeria. The parties agree to attempt amicable mediation before initiating any formal legal proceedings.
