Terms and Conditions of This Purchase Order
- This Purchase Order is issued by Northwest Wellness & Nutrition Group Inc., a corporation registered in the Province of British Columbia, Canada, and is governed by the laws of the Province of British Columbia and the federal laws of Canada applicable therein.
- The Dietitian named in this Purchase Order must hold a current, valid registration with the Dietitians Association of British Columbia (DABC) and must maintain professional liability insurance with a minimum coverage of $2,000,000 CAD throughout the duration of the service period.
- All services described in this Purchase Order shall be performed at the designated location in Canada Vancouver, specifically at 4821 West Broadway, Suite 310, unless otherwise agreed upon in writing by both parties.
- Payment shall be made within thirty (30) calendar days of receipt of a valid invoice from the Dietitian. Invoices must reference this Purchase Order number (NW-2025-04782) and include the Dietitian's BC Business License number.
- The Dietitian agrees to maintain strict confidentiality of all participant health information in accordance with the Personal Information Protection Act (PIPA) of British Columbia and the federal Privacy Act of Canada.
- Any modifications, amendments, or cancellations to this Purchase Order must be made in writing and signed by authorized representatives of both parties. Verbal agreements regarding scope changes to the Dietitian's services are not binding.
- The Dietitian shall provide all nutritional recommendations in compliance with the Canada Food Guide and current evidence-based dietary guidelines published by Health Canada.
- Northwest Wellness & Nutrition Group Inc. reserves the right to terminate this Purchase Order with thirty (30) days written notice if the Dietitian fails to meet the performance standards outlined in the service agreement or if registration with the DABC is suspended or revoked.
- All deliverables, including meal plans, workshop materials, and progress reports produced by the Dietitian under this Purchase Order, shall become the intellectual property of Northwest Wellness & Nutrition Group Inc. upon full payment.
- This Purchase Order is valid for a period of one hundred eighty (180) days from the date of issue. If services have not commenced within this period, the Purchase Order shall be considered void unless renewed in writing.
