Description of Services – Dietitian EngagementThis Purchase Order is issued by Accra Wellness & Nutrition Centre Ltd. to formally engage the services of a qualified Dietitian to provide comprehensive clinical nutrition and dietary counselling services to our clients and staff within the Ghana Accra metropolitan area. The Dietitian shall operate in accordance with the professional standards set by the Ghana Dietetic Association and the Ghana Health Service regulatory framework.

1. This Purchase Order constitutes a binding agreement between Accra Wellness & Nutrition Centre Ltd. and the named Dietitian upon signature by both parties. All services shall be rendered in and around Ghana Accra unless otherwise agreed in writing.
2. The Dietitian shall maintain valid professional registration with the Ghana Dietetic Association and shall provide proof of current licensure upon request. Any lapse in registration shall result in immediate suspension of this Purchase Order.
3. All dietary plans, client records, and nutritional data generated under this Purchase Order shall remain the exclusive property of Accra Wellness & Nutrition Centre Ltd. and shall be handled in strict compliance with the Data Protection Act, 2012 (Act 843) of Ghana.
4. The Dietitian is required to submit a detailed monthly progress report to the Ghana Accra operations manager by the 5th business day of each subsequent month. Reports must include client outcomes, workshop attendance records, and any dietary interventions performed.
5. Payment shall be made via bank transfer to the account specified by the Dietitian within thirty (30) calendar days of receipt of a valid invoice. Late payments shall attract interest at the rate of 2% per month as stipulated under Ghanaian commercial law.
6. The Dietitian shall be available for a minimum of 40 hours per week at the Ghana Accra clinic premises, with flexibility for off-site corporate engagements within the Greater Accra Region.
7. Either party may terminate this Purchase Order with thirty (30) days written notice. In the event of termination, all completed services shall be invoiced and settled in full.
8. The Dietitian shall carry professional indemnity insurance with a minimum coverage of GHS 500,000, valid throughout the term of this Purchase Order.
9. All disputes arising from this Purchase Order shall be resolved through mediation in Ghana Accra in accordance with the Mediation Act, 2010 (Act 721) of Ghana, before resorting to litigation in the courts of Ghana.
10. This Purchase Order is governed by the laws of the Republic of Ghana. Any amendments must be made in writing and signed by both parties to be valid.
Acceptance and AuthorizationBy signing below, both parties acknowledge and agree to all terms, conditions, and service specifications outlined in this Purchase Order for the engagement of the Dietitian at the Ghana Accra facility.
