Scope of Engagement

Terms & Conditions of This Purchase Order
- This Purchase Order constitutes a binding agreement between the Jerusalem Municipal Health & Wellness Authority and the Levantine Clinical Nutrition & Dietetics Center for the provision of professional Dietitian services in Israel Jerusalem.
- The Dietitian engaged under this Purchase Order shall maintain all required professional certifications, malpractice insurance (minimum coverage of 2,000,000 ILS), and compliance with the Israeli Health Professions Law, 5733-1973.
- Payment shall be processed via bank transfer to the vendor's designated account in Israel Jerusalem within thirty (30) calendar days of receipt of a valid invoice referencing this Purchase Order number (IJ-2025-DT-0047).
- The Dietitian shall submit monthly progress reports detailing the number of sessions conducted, patient outcomes, and community program participation metrics specific to the Israel Jerusalem service area.
- Either party may terminate this Purchase Order with thirty (30) days written notice. In the event of termination, the vendor shall be compensated for all Dietitian services rendered up to the effective date of termination.
- All intellectual property, including proprietary diet plans and nutritional protocols developed specifically for Israel Jerusalem residents under this Purchase Order, shall remain the property of the purchasing entity.
- This Purchase Order shall be governed by and interpreted in accordance with the laws of the State of Israel. Any disputes arising from this Purchase Order shall be subject to the exclusive jurisdiction of the competent courts in Israel Jerusalem.
- The Dietitian shall adhere to all ethical guidelines set forth by the Israeli Dietetic Association and shall maintain strict confidentiality regarding all patient nutritional information collected within Israel Jerusalem.
- Force majeure events, including but not limited to national security situations affecting Israel Jerusalem, shall temporarily suspend service obligations under this Purchase Order without penalty to either party.
- This Purchase Order may be amended only by mutual written agreement of both parties, signed and dated, referencing the original Purchase Order number.
