3.1 This Purchase Order constitutes a binding agreement between the Tel Aviv Municipal Health Authority (hereinafter "the Buyer") and Dr. Miriam Levi, Registered Dietitian (hereinafter "the Supplier"), for the provision of professional Dietitian services within the Israel Tel Aviv jurisdiction.
3.2 The Dietitian shall maintain valid professional licensure issued by the Israeli Ministry of Health and active membership in good standing with the Israel Dietetic Association throughout the entire contract period. Any lapse in licensure shall constitute grounds for immediate termination of this Purchase Order.
3.3 All services shall be performed at the locations specified in the line items above, or at such other sites within Israel Tel Aviv as mutually agreed upon in writing by both parties. The Dietitian shall bear responsibility for all travel within the Tel Aviv-Yafo municipality at no additional cost to the Buyer.
3.4 Payment terms: Net 30 days from the date of invoice submission. Invoices shall be submitted monthly and must reference this Purchase Order number (PO-TLV-DIET-2025-0472). Payment shall be made via bank transfer to the Supplier's designated account in Israel Tel Aviv.
3.5 The Dietitian shall comply with all data protection regulations under the Israeli Privacy Protection Law, 5741-1981, and shall maintain strict confidentiality of all patient and employee nutritional records. All records shall be stored in accordance with Israeli medical records retention requirements.
3.6 The Buyer reserves the right to audit the Dietitian's service delivery, documentation, and compliance with this Purchase Order at any reasonable time with 48 hours' prior written notice.
3.7 This Purchase Order is governed by the laws of the State of Israel. Any disputes arising from this Purchase Order shall be subject to the exclusive jurisdiction of the competent courts in Tel Aviv, Israel.
3.8 The Supplier shall carry professional liability insurance with a minimum coverage of ILS 1,000,000, valid throughout the contract term, and shall provide a certificate of insurance upon request.
By signing below, both parties acknowledge and agree to all terms, conditions, and service specifications outlined in this Purchase Order for Dietitian services in Israel Tel Aviv.
