4.1. This Purchase Order constitutes a binding agreement between Almaty Wellness & Nutrition Center LLP (hereinafter "the Buyer") and Dr. Serik Abenov, Dietitian (hereinafter "the Service Provider"), for the provision of professional dietary and nutritional services in Kazakhstan Almaty.
4.2. The Dietitian shall maintain all professional certifications, licenses, and registrations required by the Ministry of Health of the Republic of Kazakhstan throughout the duration of this Purchase Order. Any lapse in licensure shall be reported to the Buyer within 48 hours.
4.3. All services rendered under this Purchase Order shall be performed in accordance with the ethical standards of the National Association of Dietitians of Kazakhstan and applicable international nutrition guidelines.
4.4. The Service Provider shall maintain strict patient confidentiality in compliance with the Law of the Republic of Kazakhstan "On Personal Data and Its Protection" (No. 281-V, 2014). All patient records and dietary data shall be stored securely at the Kazakhstan Almaty premises of the Buyer.
4.5. Payment shall be made via bank transfer to the Service Provider's registered account within thirty (30) calendar days from the date of invoice submission. Late payments shall accrue interest at the rate of 0.1% per day as stipulated by the Civil Code of the Republic of Kazakhstan.
4.6. The Service Provider shall submit a detailed monthly service report to the Buyer by the 5th business day of the following month, summarizing all Dietitian consultations, meal plans issued, workshops conducted, and patient outcomes.
4.7. Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of termination, the Buyer shall compensate the Service Provider for all services rendered up to the effective date of termination.
4.8. This Purchase Order shall be governed by and construed in accordance with the laws of the Republic of Kazakhstan. Any disputes arising hereunder shall be resolved through the Almaty City Arbitration Court.
Notes: This Purchase Order is valid only for services to be delivered within the administrative boundaries of Kazakhstan Almaty. The Dietitian is expected to be physically present at the Buyer's facility for a minimum of five (5) days per week, Monday through Friday, between 09:00 and 18:00 local time (UTC+5). All dietary recommendations must account for local Kazakh food culture, seasonal availability of produce in the Almaty region, and the dietary needs of a multicultural patient population. The Service Provider shall use only approved nutritional assessment tools and software licensed for use in Kazakhstan.
