This Purchase Order is issued by Al-Nahda Health & Wellness Center, located in Kuwait Kuwait City, to formally engage the services of a qualified and licensed Dietitian for the provision of comprehensive nutritional counseling, dietary planning, and corporate wellness programming. The Dietitian, Dr. Layla Al-Sabah, shall deliver all professional services within the premises of the buyer's facility in Kuwait Kuwait City and at such other locations within the State of Kuwait as mutually agreed upon in writing. General Terms Governing This Purchase Order
- This Purchase Order constitutes a binding agreement between Al-Nahda Health & Wellness Center (hereinafter referred to as "the Buyer") and Dr. Layla Al-Sabah, Registered Dietitian (hereinafter referred to as "the Dietitian" or "the Supplier"), for the provision of professional nutritional services in Kuwait Kuwait City.
- The Dietitian shall maintain all necessary professional licenses, certifications, and malpractice insurance as required by the Kuwaiti Health Affairs Authority (KDHA) throughout the duration of this Purchase Order. Any lapse in licensure shall constitute grounds for immediate termination.
- All services shall be performed in a professional and confidential manner. The Dietitian agrees to maintain strict confidentiality regarding all employee health information obtained during the course of this engagement in Kuwait Kuwait City, in compliance with Kuwaiti data protection regulations.
- Payment shall be made within thirty (30) calendar days of receipt of a valid invoice from the Dietitian. Invoices shall be submitted in Kuwaiti Dinar (KWD) and must reference this Purchase Order number (KWT-DT-2025-0047).
- The Dietitian shall provide a minimum of fourteen (14) days' written notice for any cancellation or rescheduling of group workshops or on-site availability sessions in Kuwait Kuwait City.
- This Purchase Order may be terminated by either party with thirty (30) days' written notice. In the event of early termination, the Buyer shall be liable only for services rendered up to the date of termination.
- All disputes arising from this Purchase Order shall be resolved through amicable negotiation. Should negotiation fail, the matter shall be referred to the competent courts in Kuwait Kuwait City, State of Kuwait.
- The Dietitian shall comply with all applicable laws and regulations of the State of Kuwait, including but not limited to the Kuwaiti Labor Law, the Public Health Law, and the regulations governing the practice of dietetics and nutrition in Kuwait.
- This Purchase Order is governed by and shall be construed in accordance with the laws of the State of Kuwait. The primary place of performance of all obligations under this Purchase Order shall be Kuwait Kuwait City.
- Any amendments or modifications to this Purchase Order must be made in writing and signed by both parties. Verbal agreements shall not be valid or enforceable.
By signing below, both parties acknowledge and agree to all terms, conditions, and service specifications outlined in this Purchase Order for the engagement of the Dietitian in Kuwait Kuwait City. This document represents the complete and final agreement between the parties regarding the scope, duration, and financial terms of the Dietitian services described herein.
