Description of Services – Dietitian EngagementThis Purchase Order is issued by Golden Lotus Wellness & Nutrition Centre, Myanmar Yangon, to formally engage the services of a qualified and licensed Dietitian to provide comprehensive nutritional assessment, meal planning, and ongoing dietary counselling for our corporate wellness programme and individual client consultations. The Dietitian shall operate primarily from our facility located in Kyauktada Township, Myanmar Yangon, and shall adhere to all professional standards set forth by the Myanmar Dietetic Association.

- This Purchase Order constitutes a binding agreement between Golden Lotus Wellness & Nutrition Centre and the named Dietitian upon signature by both parties. All services shall be rendered in and around Myanmar Yangon unless otherwise agreed in writing.
- The Dietitian shall hold a valid and current professional licence issued by the relevant Myanmar regulatory authority and shall maintain professional indemnity insurance with a minimum coverage of 50,000,000 MMK throughout the duration of this engagement.
- All dietary plans and nutritional recommendations provided by the Dietitian shall be culturally sensitive, incorporating traditional Myanmar Yangon cuisine, local market availability, and the dietary preferences of the Burmese population, including considerations for Buddhist dietary practices.
- Payment shall be made within thirty (30) calendar days of receipt of a valid invoice from the Dietitian. Payment shall be processed via bank transfer to the account specified on the invoice. Late payments shall incur an interest charge of 1.5% per month.
- The Dietitian shall submit a detailed progress report to the Procurement Department of Golden Lotus Wellness & Nutrition Centre, Myanmar Yangon, on the last working day of each month. Failure to submit reports for two consecutive months shall constitute grounds for termination of this Purchase Order.
- All client data, health records, and nutritional information collected by the Dietitian during the course of this engagement shall be treated as strictly confidential and shall be handled in accordance with Myanmar data protection regulations. No data shall be shared with third parties without written consent from the client and the Centre.
- This Purchase Order may be terminated by either party with a written notice of fourteen (14) days. In the event of termination, the Dietitian shall be compensated for all services rendered up to the effective date of termination.
- Any disputes arising from this Purchase Order shall be resolved through mediation in Myanmar Yangon in accordance with the laws of the Republic of the Union of Myanmar. The courts of Myanmar Yangon shall have exclusive jurisdiction.
- The Dietitian shall comply with all applicable health and safety regulations in Myanmar Yangon, including but not limited to the requirements of the Yangon City Development Committee and the Ministry of Health and Sports of Myanmar.
- This Purchase Order is valid for a period of three (3) months from the date of issue. Any extension beyond this period shall require a written amendment signed by both parties.
