- This Purchase Order is issued by Abuja Wellness & Corporate Health Services Ltd. for the engagement of a qualified and registered Dietitian to deliver professional nutritional services to our workforce in Nigeria Abuja. The Dietitian must hold a valid license from the National Dietitians Association of Nigeria (NDAN) and a current professional indemnity insurance certificate.
- All services described in this Purchase Order shall be rendered at the company's primary office located in Garki, Nigeria Abuja, unless otherwise agreed in writing. The Dietitian is expected to be physically present on-site for a minimum of three (3) working days per week, with the remaining days available for virtual consultations and report preparation.
- The Dietitian shall comply with all applicable Nigerian health regulations, data protection laws (NDPR 2019), and professional ethical standards as set by the National Dietitians Association of Nigeria. All employee health data collected during consultations in Nigeria Abuja shall be treated with strict confidentiality.
- Payment shall be made within thirty (30) calendar days from the date of invoice submission, provided that all deliverables for the billing period have been satisfactorily completed and approved by the Requisitioning Department. Payment shall be made via bank transfer to the Dietitian's designated account in Nigeria.
- The Dietitian shall submit a detailed monthly activity report to the HR Director by the 5th business day of the following month. This report shall include the number of consultations conducted, workshops delivered, meal plans issued, and any notable health trends observed among employees in Nigeria Abuja.
- This Purchase Order is valid for a period of twelve (12) months from the commencement date. Either party may terminate this engagement with a written notice of thirty (30) days. Upon termination, the Dietitian shall complete all outstanding deliverables and submit a final comprehensive report.
- All nutritional recommendations and meal plans prepared by the Dietitian shall incorporate locally available Nigerian foods and ingredients commonly found in the Abuja market, ensuring cultural relevance and practical accessibility for employees residing in Nigeria Abuja and its environs.
- The Dietitian shall not subcontract any portion of the services outlined in this Purchase Order without prior written consent from the Buyer. Any breach of this clause shall constitute grounds for immediate termination of this Purchase Order.
- Disputes arising from this Purchase Order shall be resolved through arbitration in accordance with the Arbitration and Conciliation Act (Cap A18, LFN 2004), with the seat of arbitration in Abuja, FCT, Nigeria.
- This Purchase Order constitutes the entire agreement between the parties regarding the engagement of the Dietitian for the services described herein and supersedes all prior negotiations, representations, or agreements relating to the subject matter of this Purchase Order in Nigeria Abuja.
