This Purchase Order is issued by Capital Health & Wellness Centre, Pakistan Islamabad, to NutriCare Professional Services (Pvt.) Ltd. for the engagement of a qualified and certified Dietitian to provide comprehensive nutritional counselling, dietary planning, and clinical nutrition support services to patients and clients of the Centre. The Dietitian shall operate within the premises located in Pakistan Islamabad and shall adhere to all professional standards set by the Pakistan Dietetic Association (PDA) and the relevant regulatory authorities of the Islamabad Capital Territory.
The scope of this Purchase Order encompasses the provision of a full-time Dietitian for a period of twelve (12) months, commencing on 01 July 2025 and concluding on 30 June 2026. The Dietitian shall be responsible for conducting individual dietary assessments, developing personalized nutrition plans, managing group nutrition workshops, and providing ongoing clinical dietetic support to inpatients and outpatients at the Centre in Pakistan Islamabad.

- Delivery and Commencement: The Dietitian shall report for duty at the Capital Health & Wellness Centre premises in Pakistan Islamabad no later than 09:00 AM on 01 July 2025. All services under this Purchase Order shall be rendered exclusively at the designated location in Pakistan Islamabad unless otherwise agreed in writing.
- Qualification Requirements: The Dietitian engaged under this Purchase Order must hold a minimum of a Bachelor of Science in Nutrition and Dietetics (BSND) from a recognized university in Pakistan, along with a valid professional license issued by the Pakistan Dietetic Association. A Master's degree or higher in Clinical Nutrition is preferred.
- Payment Terms: Payment shall be made on a monthly basis within fifteen (15) working days of receipt of the invoice. The first payment shall be due by 15 August 2025. All payments shall be processed via bank transfer to the account designated by NutriCare Professional Services (Pvt.) Ltd. in Pakistan Islamabad.
- Performance Standards: The Dietitian shall conduct a minimum of twenty (20) individual patient consultations per week, prepare and update dietary plans within forty-eight (48) hours of assessment, and submit monthly progress reports to the Director of Clinical Services.
- Confidentiality: The Dietitian shall maintain strict confidentiality regarding all patient health information in accordance with the Pakistan Personal Data Protection Act and the professional code of ethics of the Pakistan Dietetic Association. All records shall be stored securely at the Pakistan Islamabad facility.
- Termination: Either party may terminate this Purchase Order with thirty (30) days written notice. In the event of termination, the Purchaser shall be liable for all services rendered up to the date of termination. The Supplier shall ensure a smooth transition of all patient dietary records to a replacement Dietitian if required.
- Dispute Resolution: Any disputes arising from this Purchase Order shall be resolved through arbitration in Pakistan Islamabad in accordance with the Arbitration Act of Pakistan. The governing law shall be the laws of the Islamic Republic of Pakistan.
- Force Majeure: Neither party shall be liable for failure to perform obligations under this Purchase Order due to events beyond reasonable control, including but not limited to natural disasters, government-imposed lockdowns in Pakistan Islamabad, or public health emergencies.
This Purchase Order constitutes a binding agreement between the parties upon signature. By signing below, both parties acknowledge and agree to all terms, conditions, and obligations outlined in this document. The Dietitian services described herein shall be delivered in full compliance with the standards expected in Pakistan Islamabad's healthcare sector.
