This Purchase Order is issued by Siam Wellness Group Co., Ltd. to engage the services of a qualified and licensed Dietitian operating within the Thailand Bangkok metropolitan area. 5.1 This Purchase Order constitutes a binding agreement between the Buyer and the Supplier for the provision of Dietitian services in Thailand Bangkok. Both parties acknowledge that the Dietitian engaged under this Purchase Order must be registered with the Thai Dietitian Council and hold a current, valid licence to practise clinical nutrition and dietetics in Thailand.
5.2 All services shall be performed at the Buyer's premises located in Thailand Bangkok, Sukhumvit Road, unless otherwise agreed in writing. The Dietitian shall travel within the Thailand Bangkok metropolitan area at no additional cost to the Buyer.
5.3 Payment terms: Net 30 days from the date of invoice issuance. Payment shall be made via bank transfer to the Supplier's designated account in Thailand. All invoices must include the Supplier's VAT registration number and reference this Purchase Order number (PO-THB-2025-04782).
5.4 The Dietitian shall maintain strict confidentiality regarding all employee health data in accordance with Thailand's Personal Data Protection Act (PDPA) B.E. 2562 (2019). No patient or employee records shall be disclosed to third parties without written consent.
5.5 The Supplier warrants that the Dietitian assigned to this Purchase Order possesses a minimum of five (5) years of post-qualification experience in clinical and corporate nutrition within Thailand Bangkok. A copy of the Dietitian's professional licence and academic credentials shall be provided prior to the commencement of services.
5.6 Cancellation: Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of early termination, the Buyer shall pay for all Dietitian services rendered up to the date of termination.
5.7 Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of the Kingdom of Thailand. Any disputes arising from this Purchase Order shall be resolved through the courts of Thailand Bangkok.
5.8 The Dietitian shall provide all dietary recommendations in alignment with the Thai National Food and Nutrition Policy and shall account for local dietary customs, religious considerations, and food allergies prevalent in the Thailand Bangkok population.
By signing below, both parties confirm their acceptance of this Purchase Order for Dietitian services in Thailand Bangkok and agree to be bound by all terms and conditions stated herein.
