This Purchase Order is issued by Al Noor Wellness & Corporate Health Solutions LLC to formally engage the services of a qualified Dietitian for the delivery of comprehensive nutritional counseling, dietary assessment, and corporate wellness programming within the United Arab Emirates Dubai region. The Dietitian shall provide all services in accordance with the standards set forth by the Dubai Health Authority (DHA) and the Ministry of Health and Prevention (MOHAP) of the United Arab Emirates.

- This Purchase Order constitutes a binding agreement between the Purchaser and the Dietitian service provider upon signature by both parties. All services shall be rendered within the jurisdiction of United Arab Emirates Dubai.
- The Dietitian shall maintain valid professional registration with the Dubai Health Authority (DHA) and shall provide proof of current licensure prior to commencing any services under this Purchase Order.
- All dietary recommendations, meal plans, and nutritional assessments provided by the Dietitian must comply with the food safety and nutrition guidelines issued by the UAE Ministry of Health and Prevention and the Dubai Municipality.
- Payment shall be made within thirty (30) calendar days from the date of invoice submission, via bank transfer to the account designated by the Dietitian. Late payments shall incur a penalty of 2% per month as per UAE commercial law.
- The Dietitian shall maintain strict confidentiality regarding all employee health data, dietary information, and personal medical records in accordance with UAE Federal Decree-Law No. 45 of 2021 on the Protection of Personal Data.
- Any modifications, cancellations, or scope changes to this Purchase Order must be documented in writing and signed by both parties. Verbal agreements shall not be enforceable.
- The Dietitian shall provide a minimum of 72 hours' written notice for any rescheduling of sessions or workshops. Cancellations with less than 24 hours' notice shall be subject to a 50% fee for the affected session.
- All services under this Purchase Order shall be conducted in English and/or Arabic, as required by the corporate client, within the United Arab Emirates Dubai metropolitan area.
- This Purchase Order is governed by and shall be interpreted in accordance with the laws of the United Arab Emirates. Any disputes shall be resolved through the Dubai Courts or, at the Purchaser's discretion, through arbitration under the Dubai International Arbitration Centre (DIAC).
- The Dietitian shall carry professional indemnity insurance with a minimum coverage of AED 1,000,000 for the duration of this engagement.
The Dietitian shall commence services no later than 1 July 2025 and shall complete all deliverables, including the final corporate wellness nutrition report, by 30 September 2025. All on-site services shall be conducted at the Purchaser's premises located in Dubai Marina, United Arab Emirates Dubai, during standard business hours (Sunday to Thursday, 9:00 AM to 6:00 PM GST).
