- This Purchase Order constitutes a formal request for Dietitian professional services to be delivered within the United Kingdom Manchester metropolitan area. All services must comply with the standards set by the British Dietetic Association and the General Medical Council where applicable.
- The Dietitian, Dr. Eleanor Hartley, confirms that she holds current professional indemnity insurance with a minimum cover of £5,000,000 and that her registration with the British Dietetic Association (RD No. 68421) remains valid throughout the contract period.
- All Dietitian consultations and group sessions must be conducted at the designated United Kingdom Manchester sites listed above, unless prior written approval is granted by the Procurement Officer for alternative locations within Greater Manchester.
- Invoicing shall be submitted monthly in arrears. Each invoice must reference this Purchase Order number (PO-2025-MAN-04872) and itemise the specific Dietitian services rendered during the billing period.
- The Buyer reserves the right to adjust the quantity of Dietitian sessions by up to 15% without issuing a revised Purchase Order, provided that the total contract value does not exceed £61,803.36 (GBP) inclusive of VAT.
- Confidentiality: The Dietitian shall adhere to the Data Protection Act 2018 and UK GDPR regulations. All patient records, dietary assessments, and clinical notes generated during the provision of services in Manchester shall be stored securely and shall not be disclosed to any third party without explicit patient consent.
- Termination: Either party may terminate this Purchase Order with 30 days' written notice. In the event of termination, the Buyer shall pay for all Dietitian services rendered up to the effective date of termination.
- Dispute resolution: Any disputes arising from this Purchase Order shall be resolved in accordance with the laws of England and Wales. The courts of Manchester, United Kingdom shall have exclusive jurisdiction.
- The Dietitian shall provide a written handover report and all outstanding patient dietary plans within 14 days of the service end date or upon termination, whichever is earlier.
