This Purchase Order authorises the delivery of all listed items to the Doctor General Practitioner practice premises located at Suite 4, Level 2, 187 George Street, Sydney NSW 2000, Australia Sydney. The supplier shall ensure that all medical and clinical supplies are transported in accordance with the Australian Therapeutic Goods Administration (TGA) guidelines and the NSW Health Department's handling protocols for medical consumables. Delivery must be completed no later than 28 June 2025, between the hours of 08:00 and 16:00 AEST, to minimise disruption to the Doctor General Practitioner's patient consultation schedule. The supplier is required to provide a signed proof-of-delivery document and a valid Australian Tax Invoice (GST-registered) upon completion of the delivery.
- This Purchase Order is issued by Sydney Harbour General Practice Clinic, a Doctor General Practitioner practice registered in Australia Sydney, and constitutes a binding agreement upon acceptance by the supplier.
- All goods supplied under this Purchase Order must comply with the Australian Consumer Law (ACL), the Therapeutic Goods Act 1989 (Cth), and all applicable New South Wales state regulations governing the supply of medical products.
- The Doctor General Practitioner practice reserves the right to inspect all delivered items within five (5) business days of receipt. Any items found to be defective, expired, or non-conforming to the specifications outlined in this Purchase Order shall be replaced at the supplier's sole cost within ten (10) business days.
- Payment of $13,290.40 AUD (inclusive of 10% GST) shall be made via Electronic Funds Transfer (EFT) to the supplier's nominated bank account within thirty (30) calendar days of the date of the valid Tax Invoice, in accordance with the Net 30 payment terms specified herein.
- The supplier warrants that all medical devices and clinical consumables listed in this Purchase Order are TGA-registered or TGA-exempt as appropriate, and that all items carry a minimum shelf life of twelve (12) months from the date of delivery to the Australia Sydney practice.
- Any variation to the scope, quantity, or delivery schedule of this Purchase Order must be agreed in writing by both the Doctor General Practitioner practice manager and the authorised representative of the supplier.
- This Purchase Order is governed by the laws of the State of New South Wales, Australia Sydney, and any disputes arising shall be subject to the exclusive jurisdiction of the courts of New South Wales.
- The supplier shall maintain adequate public liability insurance (minimum $20,000,000 AUD) and professional indemnity coverage for the duration of this Purchase Order and for a period of twelve (12) months thereafter.
- All personal health information and patient-related data handled in connection with this Purchase Order shall be protected in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs).
This Purchase Order has been prepared and authorised by the undersigned on behalf of the Doctor General Practitioner practice in Australia Sydney. The supplier's acceptance of this Purchase Order shall be confirmed in writing (including via email) within three (3) business days of the date of issue. Failure to confirm acceptance within this period shall render this Purchase Order null and void.
