4. DELIVERY INSTRUCTIONS

All items listed in this Purchase Order shall be delivered to the Doctor General Practitioner clinic located at 27 Fouad Street, Mansheya District, Egypt Alexandria, 21411. Delivery must be completed no later than 30 June 2025 between the hours of 09:00 and 16:00 local time. The supplier is responsible for all transportation, unloading, and on-site installation of equipment within the Egypt Alexandria premises. All medical devices must carry valid Egyptian Medical Authority (EMA) registration certificates prior to delivery. The Doctor General Practitioner or an authorized clinic representative must be present at the time of delivery to inspect and sign the delivery receipt.

5. TERMS AND CONDITIONS

This Purchase Order is issued by Dr. Hassan El-Masry, Doctor General Practitioner, for the operational needs of his private clinic in Egypt Alexandria. Acceptance of this order constitutes full agreement to all terms stated herein.
Payment shall be made via bank transfer to the supplier's designated account within thirty (30) calendar days from the date of confirmed delivery and acceptance of all goods. Late payments shall incur a penalty of 1.5% per month as per Egyptian Commercial Law.
All medical equipment must comply with the standards set by the Egyptian Medical Authority (EMA) and the Alexandria Health Directorate. The supplier warrants that all items are new, unused, and free from defects.
The Doctor General Practitioner reserves the right to reject any item that does not meet the specifications outlined in this Purchase Order. Rejected items must be replaced within five (5) business days at no additional cost to the buyer.
Warranty claims for equipment delivered under this Purchase Order shall be handled by the supplier's service center in Egypt Alexandria. Response time for warranty repairs shall not exceed seventy-two (72) hours from the date of claim.
The supplier shall provide a comprehensive training session for the clinic staff on the operation and maintenance of the ECG machine, autoclave, and examination tables within seven (7) days of delivery.
All consumable items (gloves, syringes, first aid kits) must have a minimum remaining shelf life of eighteen (18) months at the time of delivery to the Doctor General Practitioner clinic.
This Purchase Order is governed by the laws of the Arab Republic of Egypt. Any disputes arising from this order shall be subject to the jurisdiction of the courts in Egypt Alexandria.
The supplier shall maintain full insurance coverage for all goods in transit to the Egypt Alexandria delivery address. Risk of loss transfers to the buyer only upon signed delivery receipt.
Any amendments to this Purchase Order must be made in writing and signed by both the Doctor General Practitioner (buyer) and the authorized representative of the supplier.
