2. SCOPE OF PROCUREMENT

This Purchase Order is issued by the Addis Ababa City Health Bureau to formally procure the professional medical services of a Doctor General Practitioner for the Kirkos Sub-City Primary Health Care Center located in Ethiopia Addis Ababa. The engagement of this Doctor General Practitioner is intended to ensure continuous, accessible, and high-quality primary healthcare delivery to the residents of Kirkos Sub-City in accordance with the Federal Ministry of Health's National Health Sector Transformation Plan (NHSTP II, 2020–2025). The Doctor General Practitioner shall provide comprehensive outpatient consultations, preventive health screenings, chronic disease management, maternal and child health services, and basic diagnostic assessments within the designated facility in Ethiopia Addis Ababa.

4. TERMS AND CONDITIONS

Compliance with Ethiopian Law: This Purchase Order is governed by the laws of the Federal Democratic Republic of Ethiopia, including the Ethiopian Medical Council Act (Proclamation No. 1007/2017), the Public Procurement and Property Administration Proclamation (No. 1160/2019), and all applicable regulations of the Addis Ababa City Health Bureau.
Licensure and Registration: The Doctor General Practitioner shall maintain valid registration with the Ethiopian Medical Council and the Ethiopian Medical Association throughout the entire contract period. Any lapse in licensure shall constitute grounds for immediate termination of this Purchase Order.
Service Standards: The Doctor General Practitioner shall adhere to the Ethiopian Standard Treatment Guidelines (STG) and the National Primary Health Care Package as issued by the Federal Ministry of Health. Clinical quality audits shall be conducted quarterly by the Addis Ababa City Health Bureau Quality Assurance Unit.
Payment Schedule: Payment for the Doctor General Practitioner's services shall be disbursed monthly within thirty (30) calendar days following the end of each service month, via bank transfer to the account designated by the service provider. The first payment shall be processed within thirty days of the official commencement date.
Working Hours and Availability: The Doctor General Practitioner shall be available at the Kirkos Sub-City Primary Health Care Center from 07:30 to 15:30, Monday through Saturday, with a one-hour lunch break. Emergency on-call duties shall be rotated among the facility's medical staff as per the Addis Ababa Health Bureau duty roster.
Confidentiality and Data Protection: The Doctor General Practitioner shall comply with the Ethiopian Data Protection Proclamation (No. 1104/2019) and maintain strict patient confidentiality. All patient records shall be stored in the Addis Ababa Health Information Network and shall not be disclosed to any third party without written consent.
Termination: Either party may terminate this Purchase Order with thirty (30) days' written notice. The Addis Ababa City Health Bureau reserves the right to terminate immediately in cases of gross professional misconduct, fraud, or repeated failure to meet clinical performance benchmarks.
Dispute Resolution: Any disputes arising from this Purchase Order shall first be addressed through internal mediation by the Addis Ababa City Health Bureau. If unresolved within thirty days, the matter shall be referred to the competent court of Addis Ababa, Ethiopia, in accordance with Ethiopian civil procedure law.
Force Majeure: Neither party shall be liable for failure to perform obligations due to events beyond reasonable control, including but not limited to natural disasters, epidemics declared by the Ethiopian Public Health Institute, or government-mandated closures in Addis Ababa.
Amendments: No modification to this Purchase Order shall be valid unless executed in writing and signed by both parties. All amendments must be approved by the Addis Ababa City Health Bureau Procurement Committee.

5. AUTHORIZATION AND SIGNATURES

By signing below, both parties acknowledge and agree to all terms, conditions, and obligations set forth in this Purchase Order for the engagement of a Doctor General Practitioner at the Kirkos Sub-City Primary Health Care Center in Ethiopia Addis Ababa. This document constitutes a binding procurement instrument under Ethiopian public procurement law.
