All items covered under this Purchase Order shall be delivered to the Doctor General Practitioner clinic premises at 45 Agripas Street, Jerusalem, Israel, no later than 28 June 2025. The supplier is responsible for all transportation, insurance, and handling costs from the supplier's facility in Petah Tikva to the designated delivery location in Israel Jerusalem. Installation, calibration, and initial commissioning of all Class II and Class IIa medical devices shall be performed by the supplier's certified biomedical engineers. A written installation report, including calibration certificates and warranty documentation, must be submitted to the Doctor General Practitioner department head within five (5) business days of completion. The supplier acknowledges that the delivery location is within the Jerusalem municipal boundary and must comply with all local traffic and parking regulations applicable to commercial deliveries in Israel Jerusalem.

  • 6.1 This Purchase Order is governed by the laws of the State of Israel. Any disputes arising from this Purchase Order shall be subject to the exclusive jurisdiction of the courts in Jerusalem, Israel.
  • 6.2 All medical devices and equipment must carry valid Israeli Ministry of Health registration and SII (Israel Standards Institute) certification prior to delivery. The Doctor General Practitioner team reserves the right to reject any item that does not meet these regulatory requirements.
  • 6.3 Warranty: All equipment shall carry a minimum manufacturer's warranty of two (2) years from the date of installation. The supplier shall provide on-site repair service within Israel Jerusalem within forty-eight (48) hours of a service request.
  • 6.4 Payment: The total amount of ILS 713,428.00 shall be paid within thirty (30) calendar days of receipt of a valid tax invoice and successful acceptance of all delivered items by the Doctor General Practitioner department. Payment shall be made via bank transfer to the supplier's designated account.
  • 6.5 Late Delivery Penalty: For each calendar day of delay beyond the required delivery date, the supplier shall be liable for a penalty of 0.5% of the total Purchase Order value, up to a maximum of 10%.
  • 6.6 The supplier warrants that all items are new, unused, and free from defects in materials and workmanship. Refurbished or second-hand items are strictly prohibited under this Purchase Order.
  • 6.7 This Purchase Order may only be modified or amended in writing, signed by both the authorized officer of the Jerusalem Community Health Center and the authorized representative of the supplier.
  • 6.8 The supplier shall maintain full insurance coverage for all goods in transit and during installation at the Israel Jerusalem delivery site.
