- This Purchase Order is issued by Al-Nakheel Medical Supplies & Equipment Co. W.L.L. on behalf of Dr. Layla Al-Sabah, Doctor General Practitioner, operating Salmiya Family & General Medicine Clinic located in Kuwait Kuwait City. All goods and services listed herein are subject to the terms stated in this document.
- The supplier shall deliver all items to the specified address in Kuwait Kuwait City no later than 28 June 2025. Late delivery beyond the stipulated deadline shall incur a penalty of 1% of the total Purchase Order value per day of delay, up to a maximum of 10%.
- All medical devices and equipment must comply with the regulations set forth by the Kuwait Ministry of Health (MOH) and the Kuwait Food and Drug Authority (KFDA). The Doctor General Practitioner reserves the right to reject any item that does not meet the required regulatory standards for use in Kuwait Kuwait City.
- Payment terms: 50% advance payment upon confirmation of this Purchase Order, and 50% balance payment within 30 days of successful delivery, installation, and acceptance of all goods at the practice location in Kuwait Kuwait City.
- All equipment carries a minimum warranty of 24 months from the date of delivery. The Doctor General Practitioner shall notify the supplier in writing within 7 business days of discovering any defect or non-conformity.
- The supplier is responsible for all customs clearance, import duties, and logistics costs associated with delivering the goods from the UAE to Kuwait Kuwait City. The Purchase Order total is inclusive of all such charges.
- Installation of the examination tables, medical gas outlet kit, and electronic medical records software shall be performed by certified technicians at the practice premises in Kuwait Kuwait City. The Doctor General Practitioner or a designated representative must be present during installation for sign-off.
- This Purchase Order is governed by the laws of the State of Kuwait. Any disputes arising from this transaction shall be resolved through the competent courts in Kuwait Kuwait City.
- The supplier shall provide a detailed invoice referencing Purchase Order number PO-KWC-2025-04871 for all billing purposes. Invoices not referencing this Purchase Order number will not be processed by the accounts department.
- Confidentiality: All patient data, practice operational details, and proprietary information shared in connection with this Purchase Order shall remain strictly confidential and shall not be disclosed to any third party without the written consent of the Doctor General Practitioner.
