- This Purchase Order is issued by Dr. Aung Myint Thura, a licensed Doctor General Practitioner operating a private medical clinic in Myanmar Yangon, for the procurement of medical equipment, consumables, and software necessary for the continued operation of the general practice facility.
- All items listed in this Purchase Order must be new, unused, and in original manufacturer packaging. The supplier warrants that all medical devices comply with the Myanmar Medicines and Medical Devices Authority (MMMDA) registration requirements and are approved for use by a Doctor General Practitioner in a primary care setting within Myanmar Yangon.
- Delivery of all goods shall be made to the clinic address located at 88 Anawrahta Road, Kamayut Township, Myanmar Yangon. The supplier is responsible for all transportation, handling, and installation of equipment within the Yangon metropolitan area. Delivery must be completed no later than 30 June 2025.
- Payment shall be made within thirty (30) calendar days from the date of confirmed delivery and acceptance of all items listed in this Purchase Order. Payment will be processed via bank transfer to the supplier's designated account in Myanmar Kyat (MMK). The clinic administrator will issue a formal invoice upon delivery confirmation.
- The Doctor General Practitioner reserves the right to inspect all delivered items within five (5) business days of receipt. Any items found to be defective, damaged, or not conforming to the specifications outlined in this Purchase Order must be replaced or refunded at the supplier's sole expense within ten (10) business days.
- All medical equipment covered under this Purchase Order shall carry a minimum manufacturer warranty of two (2) years from the date of installation. The supplier shall provide a written warranty certificate for each major device (Items 01, 02, 04, and 05) to be retained by the Doctor General Practitioner for regulatory compliance records in Myanmar Yangon.
- The supplier acknowledges that the goods procured under this Purchase Order are intended exclusively for use in a licensed general practice medical facility in Myanmar Yangon and shall not be resold, transferred, or used for any purpose other than patient care by the named Doctor General Practitioner.
- In the event of a dispute arising from this Purchase Order, both parties agree to first attempt resolution through good-faith negotiation. Should negotiation fail, the matter shall be submitted to the Yangon District Court in Myanmar Yangon for adjudication under the laws of the Republic of the Union of Myanmar.
- This Purchase Order constitutes the entire agreement between the buyer and the supplier regarding the procurement of the items described herein. No modifications, amendments, or additions to this Purchase Order shall be valid unless made in writing and signed by both parties.
- The supplier shall maintain full insurance coverage for all goods in transit to the Myanmar Yangon delivery address. Risk of loss or damage transfers to the buyer only upon written confirmation of delivery and acceptance by the clinic administrator.
